Pursuant to the California Public Records Act(Government Code§7920.000 et seq.),I request copies of the following public records:
The complete GO 159A Notification Letter(with all attachments,including"Attachment A")that AT&T Mobility/New Cingular Wireless PCS,LLC
claims to have sent to the City of Lake Elsinore City Clerk's Office,dated on or about October 16,2020,referencing Site ID CLV0342, 16401
Lakeshore Dr.,Lake Elsinore,CA 92530,APN 379-250-044.
Any proof of mailing,certified mail receipt,date-stamped intake record,or correspondence log confirming the City Clerk's Office actually received this
letter in 2020.
Copies of the same GO 159A notification letter and Attachment A if separately received by the City Manager's Office(Attn:Grant Yates)and the
Community Development Department/Planning Division,as GO 159A requires separate service on each office.
Any internal correspondence,memoranda,or records referencing receipt,review,or filing of this GO 159A notification for Site ID CLV0342.
The City's document retention/tracking log or index entry(if any)reflecting when this letter was logged into the City's records system.
If the City has no responsive records for any item above,please state so explicitly in writing,identifying which office(s)were searched,consistent with
Government Code§7922.000.
Please provide these records or a written response within 10 calendar days as required by Government Code§7922.535. If any records are
withheld,please identify the specific statutory exemption relied upon per Government Code§7922.000.
