Dear Nicole:
The City is in receipt of your resignation letter dated April 11, 2022. Per our discussion, your last
day in the office will be Thursday, May 5, 2022 and your last day on payroll will be Friday, May 6,
2022. You will use a vacation day to cover May 6th.
FINAL PAY
Your final paycheck will be direct deposited on the next payroll check date of May 12, 2022.**
This check will cover all time worked between April 23, 2022 and May 6, 2022. In accordance
with City policy and relevant state law, you will also receive a payout of any accrued but unused
CAL and Admin hours.
**Note: Under Labor Code Section 220 (a provision of Labor Code section 201), the City of Lake
Elsinore is exempt from the 72-hour final pay requirement. The City however must issue a final
paycheck to the employee within a reasonable time and pursuant to policy and procedure.
Labor Code Section 201
The Legislature enacted Labor Code section 201 to ensure that employers comply with the laws
governing the payment of wages when an employment relationship ends. Section 201 provides
for immediate, or depending on the circumstances, final payment of wages and benefits within
72 hours.
Labor Code Section 220
Labor Code section 220(b) provides and exemption to municipal corporations, specifically section
220(b) provides: “Sections 200 to 211, inclusive, ad Sections 215 to 219, inclusive, do not apply
to the payment of wages of employees directly employed by any county, incorporated city or town
or other municipal corporation. All other employments are subject to these provisions.”

Health Insurance: Coverage will end June 30, 2022*
Dental Insurance: Coverage will end May 31, 2022*
Vision Insurance: Coverage will end May 31, 2022*
Life Insurance: Coverage will end May 6, 2022
LTD Insurance: Coverage will end May 6, 2022
Colonial Insurance: Coverage will end May 6, 2022
Flex Medical: You will be able to seek reimbursement through December 31, 2022 for expenses
incurred up through May 6, 2022.
Mission Square Deferred Compensation: Your money will remain in this account until you
transfer it to a new company, or you request disbursement.
Mission Square Loan: You will need to reach out to Mission Square (800-669-7400 or log into
your account) to set up payments from your bank account.
CalPERS Retirement: See attached CalPERS information.

Return of Company Property
All City property shall be returned May 5, 2022 unless otherwise discussed and agreed upon.
2022 TAX DOCUMENTS
After separation from the City, Paychex still allows you access to all your current and future tax
documents for up to 18 months. To access your 2022 documents next tax season, simply log in
to Paychex the same way you do now. You may still receive mailed paper documents as well so
if your address changes, please be sure to update that in Paychex.
