ELECTRIC LockoutlTagout Procedure Lockout/Tazout Procedure E.I.E. Electric in providing a safe workplace for its employees requires all electrical, mechanical,hydraulic, pneumatic, or other energy operated equipment to be locked out, tagged out, or blocked out to protect against inadvertent operation when the unexpected energizing or startup of the equipment, or release of stored energy,could cause injury to employees. This includes, but is not limited to, the servicing and maintenance of machines and equipment. Lockout/tagout practices are considered essential for worker safety when working on or around powered machinery and equipment. Machinery or equipment capable of movement shall be stopped and the power source de-energized,disengaged, and if necessary, the movable parts shall be mechanically blocked or locked to prevent inadvertent movement during cleaning,servicing or adjusting operations. Lockout is the preferred method of isolating machines or equipment from energy sources. If the application of a locking device is not possible, the machine or equipment shall be tagged and blocked out. 1. Definitions A. Lockout means the use of a mechanical lock device and identification tag. B. Tagout means the use of a tag only without any mechanical lock device. C. Blackout means the use of a blocking device to prevent inadvertent movement of gears,chain drives,dies,rains, or other mechanisms,which could shear or crush. 2. Set-up Procedure This procedure shall be used during servicing, set-up and/or maintenance if either of the following occurs: A. An authorized person is required to remove or bypass a guard or other safety device B. The person performing repairs must place any part of his or her body into an area on a machine or piece of equipment where a hazard of shearing or crushing may occur. C. ELECTRIC LockoutlTagout Procedure when locked out or tagged out. 2. Set-up Procedure(continued) D. This procedure does not apply to work on cord and plug connected electrical equipment that can be controlled by unplugging the equipment from the energy source and by keeping the plug under the exclusive control of the employee performing the servicing or maintenance. E. Locks and keys that are used for locking out machinery and equipment shall not be duplicated or used for any other purpose. They shall be readily identified as to intended purpose by color, style, size,etc. 3. Employee Procedure Authorized employees will accomplish all of the following steps when locking out and blocking out equipment. A. Preparation for Lockout/Tagout For each piece of equipment that is to be locked out or tagged out, the person applying the lockout/tagout shall make a survey to locate and identify all energy isolating devices in order to determine which switches, valves or other devices that apply to the equipment or machine are to be locked or tagged out. More than one energy source(electrical, mechanical, hydraulic or others)may be involved, B. Sequence of Lockout/Tagout/Blockout System Procedure Notify department supervisor and all affected employees that a lockout/tagout system is going to be used, identifying the machine or equipment and the reason for the lockout/tagout. The authorized employee shall know the type and magnitude of energy that the machine or equipment uses and understand the hazards. The employee shall not attempt a lockout/tagout without this knowledge,but should instead request assistance from a supervisor. ELECTRIC LockoutlTagout Procedure • With the machine stopped and off, operate the energy isolation device(disconnect switch, valve or other device) that will isolate the equipment from its energy source(s), such as electrical service, hydraulic power unit, air lines, etc. B. Sequence of Lockout/Tagout/Blockout System Procedure(continued) • Stored energy(such as that in springs, elevated machine members, rotating flywheels, hydraulic or pneumatic systems, gas, steam or water pressure, etc.) must be dissipated or restrained by methods such as reposition, blocking,bleeding down, etc.,before any work is attempted on the machine. • Attach individually assigned locks, tags, and hasps to all energy isolating devices. 0 Complete all required information on the tag. 0 In the space provided, print the name of the authorized employee implementing the LockoutTagout/Blockout procedure. This is the person responsible for the machine while the lockout/tagout is in effect. • In the space marked "Remarks",briefly describe the reason for the LockoutfTagout/Blockout. • Attach tags to all locks,hasps or energy isolation devices as may be appropriate to afford the best energy isolation protection for employees. • After ensuring that no personnel are exposed, and as a check on having disconnected the energy sources,operate the push button or other normal operation controls to make certain the machine or equipment will not operate. CAUTION: Remember to follow all safety procedures for the machine or equipment while conducting this test and return operating controls to "neutral" or "off' position after the check. When the equipment is locked, tagged and/or blocked out, service or maintenance may be performed. C. ELECTRIC LockoutlTagout Procedure • After the servicing and/or maintenance is complete and equipment is ready for normal production operations, check the area around the machine or equipment to ensure that no one is exposed to injury hazards. C. Restoring Machines or Equipment to Normal Production Operations (cont.) After all tools have been removed from the machine or equipment, guards have been reinstalled and employees are in the clear, remove all lockout/tagout/blockout devices. Operate the energy/isolating devices to restore energy to the machine or equipment. D. Procedure Involving More Than One Authorized Person • In the preceding steps, if more than one individual is required to lockout/tagout/blockout machinery or equipment, each shall place his/her own personal lockout device or tagout device on the energy isolation device(s)so that all lockout devices or tags must be removed from the energy isolation device before the equipment is operated. When an energy isolating device cannot accept multiple locks or tags,a multiple lockout or tagout device(hasp)may be used. Typical rebar installation of tilt-up panels were inspected per Detail 1/SO.2 with regard to rebar installation above/below an opening and in open panel areas as well as added corner bars at openings and additional perimeter reinforcement. Jamb reinforcement was inspected per Tilt- Up Jamb Schedule in addition to Tilt-Up Elevation Keynotes and Elevation Notes on sheet S1 .4. with consideration taken in regard to installation of ties and proper rebar spacing. It was noted with contractor in multiple areas that proper spacing of 3/4" clearance from form work was not being met as well minimum bar spacing of 1" for #5 bars parallel to one another in the same layer as specified in ACI-318 7.6.1 and must be addressed prior to placement of concrete. Along with this, it was noted that all tie wire should be cleaned up and properly secured to avoid wires from coming in contact with form work.
