INTRODUCTION This report has been prepared by LOR Geotechnical Group, Inc. (LOR) to present a summary of the compaction testing, materials testing, and special inspection services provided by this firm during construction of the new Mission Trail Apartments. This multi- family residential development is located at 32585 Mission Trail, in the City of Lake Elsinore, California. Used for reference during our work on this project were various onsite and offsite improvement plans(Mikami, 2018), the preliminary geotechnical investigation report(LOR, 2016), the rough grading compaction and materials testing report (LOR, 2019), and applicable portions of the California Building Code (CBC, 2016). Jurisdictional Agency: City of Lake Elsinore SCOPE The compaction testing, materials testing, and special inspection services presented herein were provided at the request of the site superintendent appointed by Lake Elsinore CCR, LLC. The scope of services we provided during the aforementioned construction period included: 6121 Quail Valley Court ♦ Riverside, CA 92507 ♦ (951) 653-1760 ♦ (951)653-1741 (Fax) ♦ www.lorgeo.com Lake Elsinore CCR, LLC Project No. 33310.84 March 18, 2020 • Periodic observation and compaction testing services during remedial grading, utility trench backfill, and preparation of subgrade/base grade for street improvements. • Obtaining soil samples for laboratory testing as necessary in support of the field compaction testing operations. • Providing continuous observation, compaction testing, and materials sampling during the asphalt concrete paving of the offsite street improvements areas. • Laboratory quality compliance testing of select asphalt concrete construction materials delivered to the project. • Periodic geotechnical observations of the foundation excavations prior to the placement of reinforcing steel and/or Portland cement concrete (PCC) materials to verify that the excavations have been properly constructed, including compaction testing of footing bottoms to verify consistency with conditions presented within the project geotechnical investigation report. • Providing Special Inspection by International Code Council certified inspectors during construction of building foundations. Inspections were provided during rebar placement, post tension tendon placement (and tensioning), concrete placement, and during post-installed anchor installation. • Sampling and testing of ready-mix concrete delivered to the site and used to construct building foundations and site improvements. Quality assurance testing was provided at the time of sampling to verify compliance with project specifications. • Laboratory testing to determine compressive strength of cylindrical concrete test specimens that were prepared and cured in accordance with practices established by the American Concrete Institute (ACI). • Preparation and submittal of daily field reports outlining the construction activities observed, compaction testing,and the laboratory materials testing reports received. • Preparation and submittal of this compaction and materials testing report following the notice of completion. 2 LOR GEOTECHNICAL GROUP,INC. Lake Elsinore CCR, LLC Project No. 33310.84 March 18, 2020 LABORATORY TESTING Compaction Characteristics Laboratory compaction characteristics were determined in accordance Standard ASTM D1557-12e1. The degree of soil compaction required to achieve the desired engineering properties is often specified as a percentage of the modified maximum dry unit weight as determined using this test method. The results of our laboratory testing provided the basis for determining relative compaction and water content of soil materials encountered during earthwork. Results of our laboratory compaction characteristics testing for soil materials encountered during our work on this project are presented within Enclosure A-1, Appendix A. Portland Cement Concrete Portland Cement Concrete (PCC) materials were used to construct building foundations, retaining walls, cast-in-place manholes, junction structures, head walls, and wing walls. Each contractor using PCC materials to construct site improvements selected PCC material suppliers at their own discretion. PCC materials supplied to the subject development were produced by Robertson's Ready-Mix and Cemex. PCC materials were sampled during placement in general accordance with procedures established by the American Concrete Institute (ACI). For quality assurance, the sampled concrete materials were tested for temperature and slump. Quality assurance test results are attached within Enclosures A-2 through A-25, in Appendix A. Compressive strength specimens were prepared and cured in accordance with practices established by the American Concrete Institute (ACI). Laboratory testing indicated that sampled PCC materials used to construct storm drain improvements did not meet the designed compressive strength after a 28 day curing period (fc). Additional compressive strength testing was performed after a 56 day cure period. Results indicated the designed compressive strength was met/exceeded after a 56 day cure period. These results were reported under separate cover for Riverside County Flood Control storm drain improvements (LOR, 2020). All other compressive strength testing performed for the subject development met/exceeded the minimum compressive strength specified in the project documents. Compressive strength results are attached within Enclosures A-2 through A-25, in Appendix A. 3 LOR GEOTECHNICAL GROUP,INC. Lake Elsinore CCR, LLC Project No. 33310.84 March 18, 2020 PCC materials used for construction of storm drain components were approved for use by others. Asphalt Concrete Quality Compliance Sampling and laboratory quality compliance testing were conducted on select base course and finish course asphalt concrete supplied to the subject development. Asphalt concrete materials sampled and tested were noted to be '/2" and 3/4" maximum medium, Caltrans Type A mixes. These materials were supplied by California Commercial Asphalt, Lake Elsinore plant. Samples were obtained in accordance with the California Test Method (CTM 125-16) during paving. Asphalt concrete materials used within the subject development were approved for use by others. The following laboratory tests were completed to verify that sampled asphalt material complies with the Caltrans Standard Specifications, section 39: gradation (CTM 202-11), density(CTM 308-10), stability(CTM 366-00), and extraction (CTM 382-14). Results of our laboratory testing indicate that the asphalt material used within the subject development complies with applicable Caltrans standards. Our laboratory test results for asphalt used within the subject development are presented within Enclosures A-26 and A-27, Appendix A. GENERAL EARTHWORK AND COMPACTION TESTING PROCEDURES All services provided by this firm during construction of the subject development were done at the request of the project superintendent. As such, a representative of this firm was onsite periodically during remedial earthwork, underground utility trench backfill, and preparation of street improvements. Geotechnical services provided by this firm during construction of the subject development included observation during earthwork, materials sampling, and compaction testing. Compaction testing was conducted in accordance with ASTM D6938-15, the Nuclear Gauge Method, and/or ASTM D1556-15, the Sand Cone Method. Relative compaction resultswere determined in accordance with ASTM D1557-12e1. Where compaction testing resulted below the minimum specified, the area was reworked by the contractor and retested until the minimum specified relative compaction was achieved. Fill soils utilized during the earthwork phases of the project were those onsite soils stockpiled during removals and/or trench excavations, and soils imported onto the site from 4 LORGEOTECHNICAL GROUP, INC. These soils were cleaned of deleterious debris, rock and/or other irreducible material with a maximum dimension of 6-inches prior to being used as fill. Fill was generally spread in 6 to 8-inch loose lifts, each lift brought to near optimum moisture content, and compacted to achieve the desired relative compaction. Compaction was achieved by various contractors for this project. Equipment used for earthwork tasks were chosen at the discretion of each contractor. Generally, heavy earthmoving equipmentwas used to excavate, transport, and compactfill soils. Excavators with compaction oriented attachments were used for various utility trench backfills. Light earthmoving equipment, steel drum rollers,and/or powered hand-operated equipmentwas used during preparation of parking lot and street improvements. Elevations and locations utilized in the determination of the removal/over-excavation limits were based on field surveys done by others. Line and grade for all improvements were verified by others. Field reports were written daily by our field representatives and submitted promptly to our client's representative.These written daily reports contain a detailed description of the day- to-day operations and the information provided within this report. Foundation Excavation Observation Following rough grading of the subject development, this firm provided periodic footing trench excavation observations as requested by the project superintendent. Footing trench excavation observations were provided forthe proposed building foundations, retaining wall foundations, and the trash enclosure foundation. All footing excavations observed by this firm appeared to have been constructed within grading limits, exposing engineered compacted fills placed during rough grading. The foundation dimensions were randomly measured for proper width and depth and were observed to meet the minimum dimensions recommended in the project preliminary geotechnical investigation report (LOR, 2016). Remedial Earthwork Remedial earthwork activities included backfilling of back-cut areas for onsite retaining walls and minor repair of the slope located within the west portion of the subject development, located near the RCFC storm drain head wall. Lake Elsinore CCR, LLC Project No. 33310.84 March 18, 2020 were conducted to verify the minimum specified relative compaction had been achieved. Compaction test results are presented within the attached Compaction Test Summaries in Enclosure B-1, Appendix B. RCFC Storm Drain Improvements The RCFC storm drain improvements included the installation of reinforced concrete pipe (RCP) main line with supporting RCP laterals and PCC structures. Compaction testing was conducted on a periodic basis during backfilling of trenches used to construct the new storm drain improvements. A total of 39 compaction tests were conducted to verify a minimum of 90 percent relative compaction had been achieved. Additional information regarding RCFC storm drain improvement compaction testing is presented within the compaction and materials testing final report for storm drain improvements (LOR, 2020). Test locations and results are presented within the attached Compaction Test Summaries, in Enclosures B-2 and B-3, Appendix B. Sewer Improvements A total of 13 compaction tests were conducted within the trenches used to construct onsite sanitary sewer improvements to verify at least 90 percent relative compaction had been achieved. Compaction test results are presented within the attached Compaction Test Summaries in Enclosure B-4,Appendix B. Water Improvements A total of 17 compaction tests were conducted within the trenches used to construct onsite water improvements to verify at least 90 percent relative compaction had been achieved. Compaction test results are presented within the attached Compaction Test Summaries in Enclosure B-5, Appendix B. Dry Utility Trench Backfill A total of 10 compaction tests were conducted within the trenches used to bury onsite dry utility lines to verify at least 90 percent relative compaction had been achieved. Compaction test results are presened within the attached Compaction Test Summaries in Enclosure B-6, Appendix B. 6 LORGEOTECHNICAL GROUP, INC. Lake Elsinore CCR, LLC Project No. 33310.84 March 18, 2020 Sidewalk Improvements A total of 8 compaction tests were conducted on sidewalk subgrade located onsite and offsite to verify at least 90 percent relative compaction had been achieved. Compaction test results are presented within the attached Compaction Test Summaries in Enclosure B-7, Appendix B. Drive Lane and Parking Improvements Drive lane and parking improvements included construction of curb &gutter, ribbon gutter, cross-gutter, and the sub-grade/base grade for the proposed paved areas. A total of 78 compaction tests were conducted during preparation of parking lot improvements to verify the minimum specified relative compaction had been achieved. Compaction test results are presented within the attached Compaction Test Summaries in Enclosures B-8 through B-11, Appendix B. Offsite Street Improvements A total of 4 compaction tests were conducted during preparation of street subgrade/base grade on Mission Trail. Compaction testing was conducted to verify at least 95 percent relative compaction had been achieved. Compaction test results are presented within the attached Compaction Test Location Maps, Enclosure B-12, in Appendix B. Offsite Asphalt Concrete Paving Hot-mix asphalt (HMA) materials were used for pavement to construct areas of Mission Trail adjacent to the subject development. This firm provided compaction testing during HMA paving operations for base course and finish course pavements. In addition to compaction testing, materials sampling and laboratory testing of HMA materials were provided. Results of our laboratory testing are presented in the Laboratory Testing section of this report. Compaction of the asphalt concrete paving was accomplished by the contractor with the use of steel-wheeled vibratory rollers. Compaction testing was conducted in general accordance with ASTM D2950/D2950m-14, the nuclear gauge method, to verify a minimum of 95 percent relative compaction was achieved. 7 LORGEOTECHNICAL GROUP, INC. Lake Elsinore CCR, LLC Project No. 33310.84 March 18, 2020 A total of 12 compaction tests were conducted on the asphalt concrete pavement. The results of our asphalt concrete paving compaction tests are presented on the attached Compaction Test Summaries, within Enclosure B-13, in Appendix B. SPECIAL INSPECTION Special inspections were provided by International Code Council (ICC) certified special inspectors during the construction of the subject development. Special inspections were provided by this firm following request for services from the superintendent appointed by Lake Elsinore CCR, LLC, in accordance with the 2016 California Building Code (CBC), table 1705.3. Special inspections provided by this firm during construction of the subject development included: • Reinforcing steel (rebar) inspection of the foundations for the proposed buildings. • Post tension tendon placement and tensioning for the proposed buildings. • Reinforced concrete placement inspection and sampling for building foundations. • Post-installed anchor installation. Daily field reports were prepared by ICC certified inspector following inspections provided for each day. These reports were supplied to our client to give to the Engineer of Record (EOR) and contain detailed information regarding the day-to-day inspection activities provided. Following these inspections, daily reports were provided to the superintendent and indicate if the observed work was in conformance with project specifications and the California Building Code (CBC, 2016). 8 LORGEOTECHNICAL GROUP, INC. Lake Elsinore CCR, LLC Project No. 33310.84 March 18, 2020 SUMMARY The observations and testing conducted by this firm for this project are not a guarantee that the contractor has performed their work without error. Observations and testing services provided by this firm have been limited to the times this firm was present during the construction operations. However, based on our observations and compaction testing conducted during the times that this firm was onsite, it is our opinion that the construction operations for the site fills reported within have been performed in general conformance with the referenced project documents and are adequate for their intended use, unless otherwise noted. Conditions on the site may change due to additional grading, erosion, or other post construction factors that may take place within the site boundaries. Therefore, this report is only indicative of those conditions observed and tested, at the time of construction, as observed by this firm. This report should be considered subject to review by the controlling inspection agencies. To ensure continued infiltration capability of the infiltration areas, a program to maintain the facilities should be considered. This program should include periodic removal of accumulated materials, which can slow the infiltration and decrease the water quality. Materials to be removed from the catch basin areas typically consist of litter, dead plant matter, and soil fines (silts and clays). Proper maintenance of the system is critical. A maintenance program which meets or exceeds those developed by the local governing agency should be prepared and properly executed. The scope of our services did not include surveying, design engineering, safety engineering, or environmental studies. All of the locations and elevations utilized at the site during our observations and testing were based on field surveys done by others. Prior to the commencement of additional grading operations, backfilling or other improvements within the site, the geotechnical engineer should be notified in advance in order to provide appropriate observation and compaction testing services. 9 LOR GEOTECHNICAL GROUP,INC.
