Date of survey. October 19,2004 IV. Inspector. Charles Ay,Certified Asbestos Consultant No.93-1030 V. Structure/service requested. The project involves multiple structures at an abandoned motel site.. The client reportf that all structures at the site will be demolished For survey and reporting purposes, the sstruucum are grouped as follows: Report S-5384A. Single-family house and storage shed. A one-story single-family house is located in the northern end of the lot Most interior and exterior walls have been previously removed A five- standing,wooden storage is located to the east of the house. Report S-5384B. Units 1-& Six small, fie&-standing cottages were located at the southwestern portion of the property. 'These units are substantially demolished 'The interior and exterior walls and the roofing materials were previously removed. Only wood fiaming remains intact, with some building material debris noted at the base of the foundations. Report S-5384C. Units 8-12. Five small, free-standing cottages are located on the west side of the property,north of units 1-6 and south of the single-f n*house. These five cottages are essentially intad. Report S-5384D. Duplex units 13-15. Tbree buildings on the east side of the lot each contain two motel units. The units are numbered 13A-13B, 14A-14B, and 15A-15B. Most interior walls and roofing materials have bow previously removed The exterior walls of the buildings are essentially intact. This report pertains only to the single-family house and the storage shed. Please refer to reports S-5384B,S-5384Cy and S-5394D for results of the surveys of the other structures at the site. The demolition contractor should be aware of the possibility that presently inaccessible suspect materials may be uncovered as renovation proceeds. If such suspect materials are discovered, the suspect material should be sampled and analyzed prior to its disturbance. Similarly, where carpeting was noted, ADI lifted a smaH comer of the carpet in representative areas to determine if suspect materials are under the carpet. However, the contractor should carefully remove carpeting to determine if suspect material is present under the carpet that was not readily aeoessible at the time of the survey. VL Site survey/bulk sampling. This report pertains only to the sing*f rdly house and the storage shed. The term "none observed" refers jointly to the single-family house and the storage shed. The following observations were made during the survey of these buildings. The storage shed has wood adenorshamors and a non-suspect, concrete floor. The only suspect material noted at the storage shed is the roofing material. Bulk sampling of the roof of the shed is included in section K below. All other comments in this section refer to the single-family house. A. Acoustical cefling material. The majority of the interior ceilings have been taken down. The living room ceiling is intact and is covered with acoustical wfiming. Acoustical debris is noted on the collapsed ceilings in the other rooms. It appears that the attic was not insulated. E. EIVAC duct system insulation. The house has a floor furnace with no suspect, accessible products observed F. Ceiling tiles. None observed- G. Sprayed structural fireproofing. None observed EF. Flooring. Flooring has been removed Srom the majority of the living areas. Approximately 20 square feet of linoleum backing residue is noted on the floors scattered throughout the house. The remainder of the floors is uncovered wood sub-flooring. Analysis was performed by polarized microscopy (PL.NI) utilizing the method described in 40 C.F.R.Chapter I(1-8-87 edition)Part 763,Subpart F,Appendix A pages 293-299. Report No.S-5384A Page 3 of narrative Single-family house&storage shed The Environmental Hazard Services, Inc. laboratory is accredited in this method under the National Voluntary Laboratory Accreditation Program administered by the National Institute of Standards and Technology, United States Department of Commerce (#101882-01 and by the Department of Health Services of the State of California(#2319). VHL Condusions. The limited building survey protocol, and subsequent bulk sample laboratory analysis as reported herein,lead to the following conclusions: 1. Friable material. (E.P.A. definition of friable is that"the material, when dry, may be crumbled, pulverized, or reduced to powder by hand pressum" AHERA section 763.83. The term "friable" does not necessarily indicate that any preset damages deterioration or health hazard adsts.) Friable asbestos was identified in these construction materials: NO FRIABLE ACM WAS IDENTIFIED. 2. Non-friable material. Non-friable asbestos was identified in these construction materials: NO NON-FRIABLE ACM WAS IDENTIFIED. If applicable, the client may incorpo= this report or its conclusions into a notice pursuant to the State of California Health and Safety Code Section 25915 et seq. or into notification pursuant to SCAQMD Rule 1403.
