Pursuant to the provisions of the Freedom of Information Act(FOIA),I am hereby requesting information and copies of documents maintained by the
Lake Elsinore Police Department relating to Michael Buchanan(DOB:04/09/1956)for the past ten years.
Specifically, I am requesting copies of all records,reports,memorandums,correspondence,complaints,incident reports,investigative reports,
accident reports,interview reports,etc...relating to the above listed individual generated and/or maintained by your police department.
This request is being made as part of a standard insurance claims investigation and for no other purpose.
If you are unable to provide any portion of the records requested,please cite the specific section of the Act under which you claim an exemption.
If there are any charges in connection with this request,please contact me and a payment will be made promptly.
