A RESOLUTION OF THE PLANNING COMMISSION OF THE CITY OF
LAKE ELSINORE, CALIFORNIA, APPROVING COMMERCIAL DESIGN
REVIEW 20,14-15 FOR A 30' WIRELESS TELECOMMUNICATIONS
FACILITY DESIGNED AS A FAUX PINE TREE LOCATED AT 28290
TRELLIS LANE
WHEREAS, Core Development Services on behalf of Verizon Wireless
("Applicant"), filed an application with the City of Lake Elsinore requesting approval of
Commercial Design Review 2014-15, which is referred to herein as the "Project"; and
WHEREAS, the Developer proposes to construct a 30' Mono-pine to house
twelve (12) 6'-0" paddle antennas and a one (1) 4'0" parabolic antenna and associated
improvements on a leased portion of property located at 28290 Trellis Lane (APN 347-
1 10-076) within a lot that includes an automotive repair facility and residence; and
WHEREAS, in accordance with the California Environmental Quality Act
(California Public Resources Code Section 21000 et seq.: "CEQA"), the Staie CEQA
Guidelines (Title 14, California Code of Regulations Section 15000 et seq.: "CEQA
Guidelines"), the Project is categorically exempt from CEQA and the CEQA Guidelines
pursuant to a Class 3 categorical exemption for new construction or conversion of
small structures; and
WHEREAS, pursuant to Section 17.184.110 of the Lake Elsinore Municipal
Code, the Planning Commission of the City of Lake Elsinore has been delegated with
the responsibility of reviewing and making decisions regarding minor design review
applications in the City; and
WHEREAS, on April 21 ,2015 at a duly noticed public hearing the Planning
Commission has considered evidence presented by the Community Development
Department and other interested parties with respect to this.
NOW, THEREFORE, THE PLANNING COMMISSION OF THE CITY OF LAKE
ELSINORE, CALIFORNIA, DOES HEREBY RESOLVE, DETERMINE AND ORDER
AS FOLLOWS:
SECTION 1. The Planning Commisston has considered the Minor Design
Review for the proposed 30' Mono-pine to house twelve (12) 6'-0" paddle antennas
and a one (1) 4'0" parabolic antenna and associated rmprovements prior to making a
decision and has found it acceptable.
SECTION 2. The California Legislature has determined that certain classes of
pro.lects do not have a significant effect on the environment and are therefore exempt
from the California Environmental Quality Act (Cal. Pub. Res. Code SS 21000 ef seq.:
"CEOA") and the State Guidelines for lmplementation of CEOA (14 C.C.R SS 15000
ef seq.: the "CEQA Guidelines"). A Class 3 exemption exists for:

GENERAL CONDITIONS
1. The proposed project has been approved forthe construction of a 30'Mono-pine
telecommunication facility to house twelve (12) 6'-0" paddle antennas and a one
(1)4'0" parabolic antenna. ln addition the applicant is requesting approval of a
30'0" X 30'-0" 8'-0" tall CMU block equipment enclosure for the accessory ground
mounted equipment. The proposed project is located 28290 Trellis Lane, Lake
Elsinore and is generally located at the southeast corner of the intersection
Highway 74 and Trellis Lane (APN 347-110-076).
2. The applicant shall defend (with counsel acceptable to the City), indemnify, and
hold harmless the City, its Officials, Officers, Employees, and Agents from any
claim, action, or proceeding against the City, its Officials, Officers, Employees or
Agents to attach, set aside, void, or annul an approval of the City, its advisory
agencies, appeal boards, or legislative body concerning the Conditional Use
Permit project attached hereto.
3. The applicant shall submit a check in the amount of $50.00 made payable to the
County of Riverside for the filing of a Notice of Determination. The check shall be
submitted to the Planning Division for processing within 48 hours of the project's
approval.
PLANNING DIVISION
4. The decision of the Planning Commission shall be final fifteen (15) days from the
date of the decision, unless an appeal has been filed with the City Council
pursuant to the applicable provisions of the Lake Elsinore Municipal Code
(LEMC)
5. Commercial Design Review (CDR) 2014-15 shall become void two (2) years
following the date on which the use permit became effective, unless prior to the
expiratron, a building permit is issued and construction commenced and diligently
pursued toward completion on the site which was the subject of the use permit
application. The Community Development Director may grant an extension of
time for up to one (1) year prior to the expiration of the initial Design Review
approval. An application for a time extension and required fee shall be submitted
a minimum of one (1) month prior to the expiration date.

6. Within 30 days of project approval, the applicant shall sign and complete an
"Acknowledgement of Conditions," and shall return the executed original to the
Community Development Department for inclusion in the case records
7. Any violation or non-compliance with the Conditions of Approval shall be cause
for the revocation of this Conditional Use Permit in accordance with LEMC
Chapter 17.168.
8. Theapplicantshall atall timescomplywithSectionlT.lT6(NoiseOrdinance) of
the Lake Elsinore Municipal Code (LEMC).
9. Conditions of Approval shall be reproduced on page one of building plans
submitted to the Building Division Plan Check. All Conditions of Approval shall
be met prior to the issuance of a Certificate of Occupancy and release of utilities.
10.All site improvements shall be constructed as indicated on the submitted and
approved plans. The applicant shall meet all required development standards as
indicated by the Commercial Design Guidelines and the Lake Elsinore Municipal
Code (LEMC). Any revisions to the approved site plan or building elevations,
shall be subject to the review of the Community Development Director or his
designee. All plans submitted for Building Division Plan Check shall conform to
the submitted plans as modified by the condrtions of approval.
11.All materials and colors depicted on the plans shall be used. lf the applicant
wishes to modify any of the approved materials or colors depicted on the plans,
the applicant shall submit a proposal setting forth the modifications for review by
the Community Development Director or their designee.
12.Prior to the submission of building permits, the applicant shall submit redesigned
plans that increase the height of the Mono-Pine by no more than 5'-0" for the
purposes of adding additional screening material to minimize the visual impact of
the paddle antennas for review by the Community Development Director or their
designee..
13.1n the event the telecommunication facility ceases to operate, the applicant or
property owner shall remove the facility and convert the site to its original
condition or better at the discretion of the Community Development Director or
Designee.
14.The Applicant shall comply with all applicable City codes and ordinances.

15. The equipment shall be maintained in the state of the original establishment of
use at the discretion of the community Development Director or their designee.
BUILIDING AND SAFETY DIVISION
PRIOR TO BUILDING PERMITS
16. All construction plans must be in accordance with the 20 1 3 building codes.
However, if the buirding permits are not obtained prior to June 17, zbts, tre
plans must be resubmitted to meet all current codes.
17. Prior to issuance of building permits, the applicant shall pay all appropriate fees.
18.The applicant shall place a weatherproof 3' x 3' sign at the entrance to the project
site identifying the approved days and hours of construction activity (i.e., 7:00
A.M. - 5:00 P.M., Monday through Friday with no construction activity to occur on
saturdays, sundays or legal holidays) and a statement that complaints regarding
the operation can be lodged with the city of Lake Elsinore code Enforcement
Division (951) 67 4-3124.
ENGINEERING
PRIOR TO GRADING PERMIT
't 9. Grading/erosion control plans shall be prepared by a calif. Registered civil
Engineer and approved prior to grading permit issuance. prior to any grading, the
applicant shall obtain a grading permit and post appropriate security.
20.All grading plan contours shall extend to minimum of 50 feet beyond property
lines to indicate existing drainage pattern.
21 . The applicant shall mitigate to prevent any flooding and/or erosion downstream
caused by development of the site and/or diversion of drainage. project
hardscape areas shall be designed and constructed to provide for drainage into
adjacent landscape.
22.The applicant shall implement NPDES Best Management practices during and
after construction to prevent discharge of pollutants into the city's storm drain
system.

23. ln accordance with the city's Franchise Agreement for waste disposal &
recycling, the applicant shall be required to contract with cR&R lnc. for removaland disposal of all waste material, debris, vegetation and other rubbish
generated during cleaning, demolition, clear and grubbing or all other phases of
construction.
PRIOR TO BUILDING PERMIT
24.Access easement or agreement shall be submitted to the Engineering
Department for plan check.
PRIOR TO FINAL INSPECTION
25.Access easement or agreement shall be recorded, with a copy submitted to the
Engineering Department
LAKE ELSINORE FIRE MARSHAL
GENERAL
26.LAKE ELSINORE FIRE PROTECTION PLANNING OFFICE RESPONSIBILITY.
It is the responsibility of the recipient of these Fire Department conditions to
fonrvard them to all interested parties. The building permit number is required on
all correspondence.

2S.APPROVED SYSTEMS- All of the following conditions titled "Prior to Final
lnspection" and/or any type of fire suppression systems must be reviewed,
inspected and approved by the Lake Elsinore Fire Protection Planning prior to
Building Safety's final inspection. The Fire Department letter of conditions, job
card and approved plans must be at the job site for all inspections.
29.ADOPTED CODES & LOCAL AMENDMENTS- The following plans have been
reviewed and conditioned with requirements that correspond with the appropriate
milestones. Regardless of the conditions, all plans shall comply with the Lake
Elsinore Municipal Code, the 2013 Adopted Codes (CFC, CBC, CMC, ect.), and

all standards referenced therein. These conditions are intended to assist in code
compliance but, any required provisions not named in these conditions shall also
apply.
30. stand-by/Emergency Generator Tank permits- Applicant/developer shall be
responsible for obtaining permits from the Lake Elsinore Fire Protection Planning
Department for aboveground fuel storage tanks in accordance with The
california Fire code. At least three copies of plans and specification sheets must
be submitted to the Fire Department for review and approval prior to installation.
A copy of the tank testing label from an independent test laboratory must be
included with the plan submittal (UL 2200 &/or UL 142). lf the tank will be
installed inside of a building a 1-hr fire rating shall be provide along with
approved ventilation. Current plan check fee is $584.00 for the first tank and
$149.00 for each additional tank.
31. Gated Entrances- Gate openings shall be as wide as the minimum 20 foot
required width of the access lane(s) entering and exiting the development. Gates
shall be located at least 35 feet into the development property to allow a vehicle
to stop without obstructing traffic on the road.
32.Address at site- The site address shall be clearly posted at the job site entrance
during construction. This will enable incoming emergency equipment and
inspectors to locate the job site from the assigned street. Numbers shall be a
minimum of 24 inches in height.
PRIOR TO BUILDING FINAL INSPECTION
33. Extinguishers (Light Hazard) - lnstall a portable fire extinguisher, with a
minimum rating of 2A-108C, for every 3,000 sq. ft. and/or 75 feet of travel
drstance. Fire extinguishers shall be mounted no higher than 5 ft above finished
floor, as measured to the top of the extinguisher. Where not readily visible, signs
shall be posted above all extinguishers to indicate their locations. Extinguishers
must have current CSFM service tags affixed; or within one year of from the date
of month and year of manufacture. (NOTE: lf only a year of manufacture is
indicated, maintenance shall be due January 1st of the yearfollowing.)
34. Knox Rapid Entry Box- A rapid entry Knox Box shall be installed on the outside
of the building. lf a Knox box has already been installed provide keys to the
tenant space for inclusion in the main building Knox Box. Key(s) shall have
durable and legible tags affixed for identification of the correlating tenant space.

Special forms are available from this office for ordering the Knox Box. lf the
building/facility is protected with a fire alarm or burglar alarm system, it is
recommended that the lock box be "tamper" monitoring.
35. Display Address (Building) - Display street numbers in a prominent location on
the address side of building(s) and rear access if applicable. Numbers and letters
shall be a minimum of 6" in height for non-habitable building(s). All addressing
must be legible, of a contrasting color with the background and adequately
illuminated to be visible from the street at all hours. All lettering shall be to
Arch itectu ral Standa rds.
36. Electrical Disconnect Sign- Electrical disconnect shall be posted
"ELECTRICAL Disconnect" in clearly legible and clearly visible lettering adjacent
to the disconnect location.
37.Final Inspection- Prior to final fire approval you must be cleared by the Fire
Department. Business is not allowed to be conducted in the building prior to final
approval. Call our office to request a Fire Department inspection when you have
approved plans and have installed items as required.
