SUBJECT: Vial of Life Program
Recommendation
It is recommended that the Commission continue the Vial of Life Program and authorize the
purchase of additional product at a cost not to exceed $900.
Background
The Vial of Life Program is a national project that allows individuals to maintain vital medical
information: current medications, allergies, ailments, contact person, primary physician and advance
directive in a location that is easily accessible for emergency personnel during a health-related crisis
or accident.
The program is designed to assist first responders with finding the medical information they need to
best treat you. People complete a form and place it in a container, such as a plastic baggie or
magnetic case, with any other relevant medical history documents. Typically, this container is the n
placed on the refrigerator, which is a place first responders know to look should they need this
information.
In October 2015, the Commission approved establishment of a Vial of Life Program in the City and
authorized the purchase of Vial of Life Containers, magnetic strips and the printing of forms.
Subsequently, another 264 containers were authorized by the Commission in May 2016.
Discussion
Since launching the program in October 2015, the City has created and distributed 528 Vial of Life
containers throughout the community. The program has been well received by those who have
requested and used them. Attached is the informational sheet and form for the program.
The “vials” have been distributed by commissioners and key stakeholders to interested residents in
our community. In addition, they are distributed at City Hall, Lake Community Center, Senior Center,
Police Department and at local fire stations. So far, they have been widely successful and we are
currently out of supply.
Therefore, staff is requesting the Commission’s authorization to purchase and assemble an
additional 264 plastic snap containers and magnetic strips. The proposed costs associated with

continuing this program are:
• Plastic Containers & Magnetic Strips - $2.19 each – Total Est. Cost: $624.41
• Forms Printed & Folded – Total Est. Cost: $220
Staff is requesting the Commission approves an expenditure of up to $900 to allow for any additional
unforeseen costs related to this item.
Fiscal Impact
The estimated budget to proceed with the Vial of Life program is up to $900. Funding is
available in the Commission’s FY 17/18 budget.
