BRADY CITY MANAGER DATE: OCTOBER 27, 2009 SUBJECT: INTERCHANGE IMPROVEMENTS AT INTERSTATE 1 -15 AND CENTRAL AVENUE (SR -74) — CHANGE ORDER NO. 1 FOR SURVEYING AND CHANGE ORDER NO. 2 FOR SOILS TESTING CONSULTANTS Background On August 26, 2008, the City Council awarded a construction contract to Ortiz Enterprise, Inc. in the amount of $2,896,591.30 for the construction of improvements to the I- 15/SR -74 interchange. This priority project will provide for additional traffic lanes on the freeway ramps as well as under the freeway bridge. Construction contract documents for this project required the City to furnish surveying and soil testing activities to insure work is done in compliance with project plans and specifications. The firm of KDM Meridian was selected to do the survey work and the firm City & County Soil Engineering and Testing provided materials testing services. Discussion During construction, staff determined additional soil was required to complete the slope grading for the southbound on ramp. This change required additional testing and inspection of the slope construction which was beyond the original contract scope of work. Survey work was required to determine the additional soil volume that was needed to complete the slope grading on the southbound ramp. Additional survey was also required for the storm drain culvert extension at Dexter Avenue and verification of the bridge height for clearance over Central Avenue. This additional work was beyond the original contract scope of work. CC Agenda 10 -27 -2009 Item No. 5 Page 1 of 3 I -15 /Central — Surveying and Soil Testing CO No. 1 & No. 2 October 27, 2009 Page 2 Fiscal Impact The original cost estimate for survey work approved for KDM Meridian was $80,700; the additional survey work performed totaled $23,901.25. The soil testing costs provided by the firm City & County Soil Engineering and Testing was $53,005; the additional grading and compaction testing performed to be in conformance with Caltrans specifications was $28,520.75. Total cost for the increase is $52,422 and sufficient funds are available in the project contingency. Recommendations 1. Approve Change Order No. 2 in the amount of $28, 520.75 for City & County Soil Engineering and Testing. 2. Approve Change Order No. 1 in the amount of $23,901.25 for KDM Meridian. 3.
