BRADY CITY MANAGER DATE: APRIL 12, 2011 SUBJECT: NOTICE OF COMPLETION FINAL CHANGE ORDER LAKESHORE DRIVE STREET IMPROVEMENTS FROM VISCAYA STREET TO KANSAS STREET PROJECT NO. 4345 Background On April 14, 2009, City Council approved the contract award for professional services to Winzler & Kelly for the preparation of engineering plans and specifications for the Lakeshore Drive Street Improvements from Viscaya Street to Kansas Street. Based.on, the plans prepared by Winzler & Kelly, on September 22, 2009, City Council approved the transfer of $530,000 from the FY2008/2009 Pavement Management Program to fund the project construction, contingency, inspection, survey and geotechnical services. Staff published the project for bid and of the nine (9) bids received, City Council awarded the construction contract to the lowest bidder, Elite Bobcat Services, Inc. Discussion Elite Bobcat Services, Inc. completed the contract and change order work on March 31, 2011 in conformance with the approved plans and specifications. The asphalt cap was placed the day before the storm sequence in late December. Due to the intensity and duration of this storm the asphalt concrete cap experienced raveling and stripping of the fine aggregates. Due to this damage, the City rejected the asphalt concrete cap and requested the contractor to contact Asphalt Pavement and Recycling Technologies, Inc. (APART) to determine the exact cause of this failure and to recommend corrective measures that would assure the long term viability of the pavement. APART recommendation was to remove 2-1/2 inches of the failed asphalt areas and slurry seal the project limits to replace the fines that were stripped away and to provide a neat uniform clean appearance. The completion of slurry seal was delayed due to the temperature requirements of when placing slurry seal.
