K Project Understanding The City of Lake Elsinore is soliciting proposals for topographic survey and mapping services for the street segments: • Riverside Drive from Eisenhower Drive to Grand Avenue (Approx. Distance: 3,550 feet) • Grand Avenue from Riverside Drive to Macy Street (Approx. Distance: 1,850 feet) Within the limits of the Project, Riverside Drive is a four lane roadway with a striped median and bike lanes on both sides and a concrete sidewalk only along the Lakeside High School frontage. Grand Avenue consists of a two lane road with bike lanes on both sides and no existing sidewalks. The project objective is to prepare a base map for preparation of improvement plans for installation of missing sidewalk segments within the project limits. The total length of the project is approximately 5,400 feet. The limits of survey will extend at least 50 feet beyond the proposed improvements. The base map will consist of the street centerlines with centerline stationing, public right-of-way and property lines, elevations along cross sections at 50 ft. intervals as shown in the RFP and surface culture such as power poles, fire hydrants, bus benches and trees. Please refer to the "Scope of Work" in this proposal for additional details. Key Issues: Since the purpose of the Project is to create a base map for new sidewalk construction, it is important to establish the street right-of-way. The topographic map will show the street RIW and property lines based on the available assessors maps. However, should the City be interested in establishing the right-of-way and easements more accurately, then CNC will utilize preliminary title reports forthe parcel adjacent the roadways. According to our preliminary investigation, the proposed sidewalk improvement swill be adjacent to forty eight (48) parcels. Unless otherwise directed by the City, all centerline stationing will be arbitrarily established by CNC. CNC's Commitments CNC commits to the following. for the entire duration of this project: • The City's RFP released for this project (released on November 29, 2010) will be incorporated in its entirety as a part of CNC's Proposal. • The RFP and this proposal will jointly become part of the agreement for Professional Consultant Services for this project when said agreement is fully executed by CNC and the Mayor or City Manager of Lake Elsinore. • Services to be provided by CNC and fees therefore will be in accordance with the. City's RFP. • All charges for the Project will be "Not-to-Exceed Fee" and include reimbursable expenses; as submitted with and made a part of this proposal. Page 16 of 39 1CNC . . RING I S. . . . . . . . CO MIT RUCTION DESIGN SURVEY SERVICES FOR RIVERSIDE DRIVE SIDEWALK IMPROVEMENT SECTION 2 PROJECT UNDERSTANDING AND COMMITMENTS • CNC will document and provide the results of work to the satisfaction of the City. • CNC will immediately document and notify the City of any defects or hazardous conditions observed in the vicinity of the project sire prior, during, or after the survey work, for as long as our crew is at the site. • CNC never has nor will it discriminate against any employee or applicant for employment because of race, color, religion, sex, or national origin. • All federal laws and regulations shall be adhered to notwithstanding any state or local laws and regulations. In a case of conflict between federal, state or local laws or regulations the strictest shall be adhered to. • CNC will allow all authorized federal, state, county, and City officials access to place of work, books, documents, papers, fiscal, payroll, materials, and other relevant contract records pertinent to this special project. All relevant records shall be retained for at least three years. • CNC complies with the Davis-Bacon Fair Labor Standards Act (40 USC 276-a through a-7), and the implementation regulations issued pursuant thereto (29 CFR Section 1,5), any amendments thereof and the California Labor Code. Pursuant to the said regulations, entitled "Federal Labor Standards Provisions," "Federal Prevailing Wage Decision" and State of California prevailing wage rates respectively. + CNC will comply with the Copeland Anti-Kickback Act (18 USC 874) and the Implementation Regulation (29 CFR 3) issued pursuant thereto,.and any amendments thereof. • CNC offers and agrees to assign to the City all rights, title, and interest in and to all causes of action it may have under Section 4 of the Clayton Act (15 USC Sec. 15) or under the Cartwright Act (Chapter 2 [commencing with Section 16700] of Part 2 of Division 7 of the Business and Professions Code), arising from purchases of goods, services, or materials pursuant to the public works or the subcontract. ICON . . . . . . . . DESIGN SURVEY SERVICES FOR RIVERSIDE DRIVE SIDEWALK IMPROVEMENT SECT/ON 3 SCOPE OF WORK Based on our understanding of the project, the following Scope of Services defines specific tasks required to provide the City of Lake Elsinore with complete topographic mapping of the project as well as supporting project records and contract services. The project will be divided into two distinct work tasks, as follows: Task A: Project Management and Coordination Upon receipt of notice to proceed from the City of Lake Elsinore, CNC will arrange a kick off meeting with representatives from the City and the CNC project team. CNC will provide the City with a draft agenda for approval prior to the meeting. The following items are expected to be included on the agenda: 1. Finalize the scope of work and deliverables 2. Identify the points of contact 3. Updated project schedule prepared in Microsoft Project. 4. Confirm the horizontal and vertical controls CNC Project Manager will be responsible for preparation and distribution of the meeting minutes. The minutes will identify specific action items along with the individual or entity responsible and the deadline for completion of the task. CNC will be available to meet with the City as necessary for the duration of the project to discuss any potential delays or issues and to provide coordination. Task B: Design Survey and Base Map Preparation Controls and Centedfne Stationing: CNC will research available bench marks and monuments at the City of Lake Elsinore and County of Riverside. Unless otherwise directed by the City, we assume horizontal and vertical controls for this project will be based on State Plane Coordinates (NAD 83) and NAVD 88, respectively. CNC will research recorded tract maps and parcel maps adjacent to the project at the County and the City, as well as as-built street drawings. CNC will verify the primary horizontal and vertical controls in the field and establish new secondary controls. Based on the centerline monuments located in the field and available record drawings, CNC will establish the street centerline. The centerline stationing will be based on ground distances; NOT grid distances. Limits and Content of Survev: Field survey will consist of cross sections at 100-foot intervals, as well as at BCR's and ECR's, BC's and EC's. Additionally, elevations at the top and bottom of curbs and along the edge of pavement will be surveyed at 25-foot intervals. CNC will locate the street right-of-way (RNV) in the field as shown on the record drawings. At a minimum, each cross section will consist of the ground shot at the RIW, back of walk (BW), if different from the RIW, top of curb (TC), gutter flowline (FL); edge of gutter (EG), lane lines, legends, traffic signal loops, street signs with call-outs, grade breaks, and finish surface. Survey will be extended 100 feet past the ends of curbs returns at all intersections. Page 18 of 39 ♦CNC DESIGN SURVEY SERVICES FOR RIVERSIDE DRIVE SIDEWALK IMPROVEMENT SECTION 3 SCOPE OF WORK Survey will also include all surface culture within the project limits such as meter boxes, utility poles and valve cans, trees, grass, fences, retaining curbs, retaining walls, surface utility features such as manholes, valve covers and catch basins. This effort will show the location of surface utility features within the project limits. Task C describes the scope of work for investigation of existing underground utilities. CNC crews will survey all existing curb ramps as necessary to allow evaluation for compliance with Americans with Disabilities Act (ADA) requirements. Field survey will also include locating all existing trees within the project limits. CNC will measure each tree trunk diameter over 4" at breast height and will identify them on the map. A concrete lined channel crosses Riverside Drive approximately 2,275 feet south of Eisenhower Drive. Field survey will include top and bottom of the concrete channel at 25' intervals at least 50' in either direction from the roadway. Since the purpose of the Project is to create a base map for new sidewalk construction, it is important to establish the street right-of-way. The topographic map will show the street RIW, property lines and existing easements based on available record maps. However, should the City be interested in establishing the right-of-way and easements more accurately, then CNC will utilize preliminary title reports for the parcel adjacent the roadways. In either case, the assessor's parcel number (APN), street address and owner's name for each property adjacent to the street will be shown on the map. Based on our preliminary investigation, the proposed sidewalk improvements will be adjacent to the following forty eight (48) parcels: Grand Avenue from Riverside Drive to Macy Street. (South side of Grand) APN 387-080-03 & 04 387-090-01, 07, 46, 56, 57, 58 & 63 387-101-01,17,03 387-102-01 thru 04 387-103-06 (North side of Grand) APN 379-060-22, 05, 26, 07, 08,17, 10 & 16 Riverside Drive from Eisenhower Drive to Grand Avenue APN 379-050-41, 34, 22, 37,38 & 17 379-060-27, 29,19 & 01 379-070-07, 08 & 09 379-090-21 & 22 379-100-08, 09,10, 20 & 21 5 Page 19 of 39 AC N C . . . . . . . . . . . . . . . . . . . DESIGN SURVEY SERVICES FOR RIVERSIDE DRIVE SIDEWALK IMPROVEMENT SECTION 3 SCOPE OF WORK 379,314.01 Based on the parcel boundary information, CNC will locate and survey each property corner and right-of-way boundary. Unless otherwise directed by the City, all centerline stationing will be arbitrarily established by CNC. Unless there is a barrier at the RM, the limits of survey will be extended 20 feet beyond the RM, except over the Freeway. Ma in Survey points will be data collected and electronically downloaded to AutoCAD 2011 with the Civil 3D enhancement package to prepare the topographic mapping ata scale of 1"=20' and at 0.10-foot intervals. CNC will prepare 24" x 36" base sheets showing the topographic mapping using the City's standard AutoCAD layering theme with match lines, street names, north arrows and graphic scales consisting of street centerline with stationing and bearings and distances, contour lines, spot elevations, surface culture such as trees and fire hydrants, RM line lot lines from record drawings and existing surface and underground utilities. Refer to Task B for scope of services for utility investigation. CNC will also prepare 24" x 36" cross section sheets at 50-ft stations with dimensions from centerline to RM and curb, elevations and slopes. Scale will be 1"=40' horizontal and 1"=4' vertical. Final mapping and cross sections will be submitted in hard copy and electronic format in accordance with the requirements of the RFP. Task C: Utility Investigation and Coordination CNC will compile a list of Dig Alert member agencies in the area and augment this list based on information from the City. CNC will contact the utilities on this list and request copies of record drawings of their facilities in the vicinity of the project. All utility correspondence will be submitted to the City. CNC will maintain a utility log with the names and contact information for all the utility purveyors along with the dates and summary of all correspondence with each purveyor. CNC will review the record drawings from utilities and transfer existing facilities shown on the drawings to the base map. CNC will hire the services of a licensed contractor to pothole existing utilities and measure the top and size of the conduit.
